Pennsylvania Drivers License Name Change Information
After you've legally decided to change your name, because of a marriage, or divorce or by the authority of a court, then you must follow the instructions below. The process is very straightforward and involves filling out the name change application.
In Person

Download and print the Dl-901 Name Change application from PennDOT's website.

The form will ask you to provide your driver's license number, past full name, new name, birth date, phone number, email address, and reason for name change.

Note: If your license is expiring in less than six (6) months time, you will have to use the drivers license renewal PA instructions instead of doing step 1 above.

Map out any photo center office in Pennsylvania, and show up in person.

Prepare to show the documentation which proves your new name. The required documents differ depending on your name change situation. See below for a complete list of documents and explanation.

There is no cost to have your license updated.
By Mail

Cost for Pennsylvania Drivers License Name Change
There is no cost to apply for a change or correction to your driver's license in PA.
Documents Required
If your new name will be your name of birth, or spouse's surname, or a name ordered via court, then you must show the following:

Marriage License OR

Divorce Decree OR
-
If a court has ordered your name change, please bring the certified court order papers.
If your new name does not fall into any of the above categories, then you must show the following documents:

Either a copy of your Social Security Card, or a copy of its record AND

Two (2) copies of documents with your new name from the following list:

Selective Service Card, Tax Records, active Passport, Voter Registration, Birth Certificate, any ID from a governmental source (with photo).
